briefly describe your proposed solution to address the problem, issue, suggestion, initiative, or educational need and how it has changed since you first envisioned it.

Now that you have completed a series of assignments that have led you into the active project planning and development stage for your project, briefly describe your proposed solution to address the problem, issue, suggestion, initiative, or educational need and how it has changed since you first envisioned it. What led to your current perspective and direction?

Identify common health traditions based on cultural heritage. Evaluate and discuss how the families subscribe to these traditions and practices. Address health maintenance, health protection, and health restoration as they relate to your assessment.

The learning activity and corresponding assignment in this topic requires students to perform a heritage assessment with families selected by students from their local communities.

Click on http://wps.prenhall.com/wps/media/objects/663/679611/box_6_1.pdf in order to access the “Heritage Assessment Tool.”

Interview three families from different cultures. One family should be from your own culture. Compare the differences in health traditions between these cultures.

Assess the three families using the “Heritage Assessment Tool.” In 1,000-1,500 words, discuss the usefulness of applying a heritage assessment to evaluate the needs of families and develop plans for health maintenance, health protection, and health restoration. Include the following:

  1. Perform a heritage assessment on three families. One of these families should be from your own culture.
  2. Complete the “Heritage Assessment Tool” for each of the three families interviewed and submit the three assessments to: RNBSNclientcare@gcu.edu. You are not required to include the tool in your LoudCloud submission.
  3. Identify common health traditions based on cultural heritage. Evaluate and discuss how the families subscribe to these traditions and practices. Address health maintenance, health protection, and health restoration as they relate to your assessment.

Description of the case/situation/conditions. Use a real life patient situation or condition to describe your case, including the problems the patient encounters and the barriers to care.

Choose a specific focus of patient practice (e.g., acute care hospital, clinic, primary care, long-term care, home health). Select a particular disease process (congestive heart failure, fall, diabetes, etc.). Identify and fully describe a technology element that could be involved in providing care to a patient with your selected disease process and the patient practice. Define how this technology will integrate treatment, monitoring or communication from the identified care setting to the home and then to ongoing care through the clinic.

Your case report must include the following:

1. Introduction with a problem statement and your disease process described from the focus of patient practice.

2. Synthesized literature review.

3. Description of the case/situation/conditions. Use a real life patient situation or condition to describe your case, including the problems the patient encounters and the barriers to care.

4. Describe at least one technology that may be used as a solution to the patient situation or condition described in your case.

5. Describe how the technology can be used specifically in the case you have proposed.

6. Summary of the case integrating proposed solutions(s).

Explain how your proposal will directly and indirectly impact each of the aspects.

After discussion with your mentor, name one financial aspect, one quality aspect, and one clinical aspect that need to be taken into account for developing the evidence-based practice project. Explain how your proposal will directly and indirectly impact each of the aspects.

What is your upcoming week’s detailed schedule at your community practice experience placement?

  1. What are your upcoming week’s specific learning goals and objectives?
  2. What is your upcoming week’s detailed schedule at your community practice experience placement?
  3. Were there any placement items/issues that occurred this week that you feel your instructor should be aware of that are private in nature and are more appropriately shared here than in the discussion board with your classmates?
  4. Give a brief description of an objective you worked on this week. Make sure to cite at least one reference showing how your objective relates to the public health knowledge you’ve studied during this course or the public health course. You may choose to reference your e-text, journal articles, or videos you’ve studied during these courses or you may find an outside reference on your own to further enhance your public health knowledge and practices.

Remember, your journal entries are an important aspect of clinical learning as they serve to help you reflect upon and get the most out of your community practice experience. Therefore, your journal entry should include the who, what, where, and when of your community practice experience. Remember, this should simulate a dialogue that would normally take place face-to-face with your community practice experience instructor.

PART 3

Read the case study and provide complete and detailed responses for the questions below. Some questions will ask for information not included within this chapter. Use your textbook, a medical dictionary, or any other reference material you choose to answer these questions.  A 40-year-old female is seen in the dermatologist’s office, on the recommendation of her internist, for a workup for suspected MM on her left forearm. The suspicious lesion was a blackish skin growth approximately 1 cm in diameter. The dermatologist examined the patient and a tissue biopsy was performed. The biopsy confirmed that the growth was MM. Surgery was performed to remove the tumor. In addition, a 5 cm by 5 cm square of skin immediately surrounding the tumor was also moved. Because the removed skin included all three layers of skin, a skin graft was necessary to cover the open area. The donor site was the patient’s thigh. Following recovery from surgery, the patient was referred to an oncologist for follow-up care.  Critical Thinking Questions  Answer the questions below regarding this case study. Do not just copy words out of the case study; translate all medical terms. To answer some of these questions, you may need to look up information from another chapter of this textbook.

1.   What pathological condition does the internist think this patient may have? Look this condition up in a reference source, and include a short description of it in your answer.

2.   What diagnostic test did the dermatologist perform? Describe it in your own words.

3.   Explain why a skin graft was necessary.        4.   Explain why it was necessary to remove so much additional skin tissue along with the tumor.

5.   What is the specialty of an oncologist?        6.   What do you think the possible follow-up care might include?
