According to the American Psychological Association, Cognitive Behavioral Therapy (CBT), is a complex and evolving model of treatment has been developed for and applied to a wide range of mental and physical problems and disorders. In this case study the therapist used behavioral strategies, problem solving strategies to assess the traumatic event that the client was experiencing. (American Psychological Association, 2017)
The therapist in in the case study identified a specific problem, focused on how it was negatively impacting the client, as she expressed feelings of guilt and anger. Techniques such as reframing and restructuring were key pointers used to help change the client behavior. Although the past experience is important, the tools provided were helpful in solving the issue at hand, which is post-traumatic stress disorder.
Cognitive Behavioral therapy (CBT), can be used in my future practice, I am currently working with clients who are addicted to substances such as opioid, benzodiazepine, and alcohol. This therapy will help the client identify negative patterns and learn how reframe their thoughts and change their behavior so that they can become more productive, by turning their thoughts into positive feelings and actions.
Dialectical Behavioral Therapy (DBT), is a type of cognitive behavioral therapy that was originally developed to treat borderline personality disorder, however research shows that DBT can treat depression, bipolar disorder, binge eating, bulimia, post-traumatic stress disorder, and substance abuse. In this case, Jill was assisted by her therapist in confronting her maladaptive beliefs about the traumatic event that killed her fellow service members when they were hit by an improvised explosive device while driving a combat supply truck. It was done through Socratic questioning when she led Jill through a series of questions and allowed her to visualize the sequence of events that led to the trauma and allowed to focus her on misconception about self blame. It gives Jill a chance to assimilate the right information and change the pathologic component of the trauma memory (Stein, 2021)
Discuss how you could implement CBT into your clinical practice in the future.
I would implement Cognitive Behavioral therapy as the first line intervention with a trauma-focused psychotherapy. Multiple clinical studies have shown that a combination of exposure and cognitive therapy to be effective for PTSD (Stein, 2020)
What specific diagnoses can DBT be used with?
Per UpToDate, Dialectical behavior therapy is specific for patient with severe problems in emotional regulations, common in Borderline Personality disorder and may have a specific role for managing patients with complex presentations of PTSD (ie, those with concurrent features of personality disorder).
