Scenario

Imagine that you are faced with finding a nursing home for an elderly family member. What are some things that you should look for when choosing a quality nursing home?

Write a two to three paragraph summary indicating what you suggest looking for when selecting a quality nursing home. Cite your sources using APA style guidelines. For assistance with APA, please visit your college’s Online Library which is accessible through the Resources tab.

Do you think this shift is for the better? How do you think this team approach to care can affect a patient’s care choices?

Consider the scenarios given below and answer the questions listed after each scenario. Justify your answers with appropriate research and reasoning.

Scenario 1

In the past, paternalism among healthcare providers was an accepted way of practicing medicine. Patients sought care from physicians and accepted their opinions and treatment choices without question. However, in today’s healthcare industry, there has been a shift away from paternalism as patients become active in their treatment choices and work as a team with their healthcare professionals. In the light of these facts, answer the following questions:

What do you feel has caused this shift and why?
Do you think this shift is for the better? How do you think this team approach to care can affect a patient’s care choices?
Scenario 2

With increasing diversity in the United States, a question has arisen regarding what accommodations should be made to assist the diverse population. Some groups or individuals feel in order to address the increasingly diverse population; we must employ a medical staff that is reflective of a community’s population. For example, if half of the population in a community is Hispanic, the medical office staff should be comprised of Hispanic workers.

Do you agree or disagree with this solution for addressing diversity in healthcare and why or why not? How would you address these diversity issues of the Unites States?
In the United States healthcare system, disparities such as socioeconomic, racial, and ethnic can be found. What do you feel are the most important disparities that need to be addressed in the healthcare industry? How do the disparities affect individuals in terms of healthcare and what do you feel is the best course of action to remedy each disparity and why?

What do you feel the impact of the issue in the article will be on the healthcare industry? What can be done to ensure rationing is done fairly?

Rationing Issues in Healthcare

The concept of rationing healthcare is dealt with on a daily basis. Healthcare rationing in the United States exists in various forms. Access to private health insurance is rationed based on price and ability to pay. Those not able to afford a health insurance policy are unable to acquire one, and sometimes, insurance companies prescreen applicants for pre-existing medical conditions and either decline to cover the applicant or apply additional price and medical coverage conditions. Access to state Medicaid programs is restricted by income and asset limits through a means test and to other federal and state eligibility regulations. Health maintenance organizations (HMOs) that commonly cover the bulk of the population restrict access to treatment via financial and clinical access limits.

Using the South University Online Library, find a current article (no more than two years old) on rationing issues in healthcare. The article should also be peer-reviewed.

Note: Peer-reviewed refers to articles reviewed and approved by authors’ professional peers who are experts on the topic being discussed.

After reading your article, summarize its contents and the main theme discussed. Then, answer the following questions:

  • How is rationing defined and what criteria are offered to ration care?
  • Discuss and apply at least one of the major ethical theories to the issue and the ethical decision-making process to the issue.
  • What do you feel the impact of the issue in the article will be on the healthcare industry? What can be done to ensure rationing is done fairly?
  • Discuss the major codes of ethics of the stakeholders involved in the issue and how these codes will affect the decision-making process and the final decision.
  • Examine and discuss the impact that the issue and the final decision will have on the stakeholders involved.
  • Discuss any potential policy implications for the issue and the final decision.

Compile the summary and answers to the above questions in a 7- to 10-page Microsoft Word document.

Support your responses with examples.

Cite any sources in APA format.

Discuss the steps that will be taken to gather and present the information.

Certified medical administrative assistants (CMAAs) need to be aware of the many medical options that are available in their community.

  • For this assignment, 2 pages; APA format develop a document that contains the community resources for breast cancer patients.
  • Discuss the steps that will be taken to gather and present the information.
  • Include a procedure to update the information on a regular basis.

Why is it important for you, as a master’s-prepared nurse leader, to be cognizant of these matters?

Organizational Foundations

As you strive to grow in your leadership skills and abilities, you will find that the context in which you work influences your motivation and areas of focus. In a similar vein, your commitment to developing professionally can contribute toward organizational effectiveness.

To that end, it is critical to recognize the importance of organizational culture and climate. In particular, through this week’s Learning Resources, you may consider several questions: How do an organization’s mission, vision, and values relate to its culture? What is the difference between culture and climate? Moreover, how are culture and climate manifested within the organization?

For this Discussion, you explore the culture and climate of your current organization or one with which you are familiar. You also consider decisions and day-to-day practices and the way they relate to the organization’s mission, vision, and values.

To prepare:

  • Review the information related to planning and decision making in health care organizations presented in the textbook, Leadership Roles and Management Functions in Nursing: Theory and Application. Consider how planning and decision making relate to an organization’s mission, vision, and values, as well as its culture and its climate.
  • Familiarize yourself with the mission, vision, and values of your organization or one with which you are familiar. Consider how the statements and actions of leaders and others within the organization support or demonstrate the organizational mission, vision, and values. In addition, note any apparent discrepancies between word and deed. Think about how this translates into expectations for direct service providers. Note any data or artifacts that seem to indicate whether behaviors within the organization are congruent with its mission, vision, and values.
  • Begin to examine and reflect on the culture and climate of the organization. How do culture and climate differ?
  • Why is it important for you, as a master’s-prepared nurse leader, to be cognizant of these matters?

Post  (1) a description of your selected organization’s mission, vision, and values. (2) Describe relevant data, or artifacts, words, and actions of leaders and others in the organization that support, or perhaps appear to contradict the organization’s mission, vision, and values statement. (3) In addition, discuss the organization’s culture and its climate, differentiating between the two. (4) Explain why examining these matters is significant to your role as a nurse leader

What questions have you thought about in a particular area of care such as a  procedure or policy?

Nursing Research and Evidence-Based Practice

In your practice as a nurse, you may use procedures and methods that did not necessarily originate in evidence, but instead were derived from informal and unwritten conventions, traditions, and observations. While these techniques may have merit, practices are constantly being updated and contradicted by information from scholarly research studies and professional guidelines. This new information serves as “evidence” for revising practices to improve outcomes across health care.

Based on this evidence, you can formulate a question. In this Discussion, you consider the use of evidence-based practice in your own organization and formulate a question that you will need to answer for your portfolio project. This is called a PICOT question. You will also investigate strategies for overcoming barriers to implementing evidence-based practice (EBP).

To prepare:

  • Consider a recent clinical experience in which you were providing care for a patient.
  • Determine the extent to which the care that you provided was based on evidence and research findings or supported only by your organization’s standard procedures. How do you know if the tasks were based on research?
  • What questions have you thought about in a particular area of care such as a  procedure or policy?
  • Review Chapter 2, pages 36—39 on “Asking Well worded Clinical Questions” in Polit & Beck and consult the resource from the Walden Student Center for Success: Clinical Question Anatomy & examples of PICOT questions (found in this week’s Learning Resources). Formulate your background questions and PICOT question.
  • Reflect on the barriers that might inhibit the implementation of evidence-based practice in your clinical environment.
  • Review the article “Adopting Evidence-Based Practice in Clinical Decision Making” in this week’s Learning Resources. Select one of the barriers described that is evident in your organization and formulate a plan for overcoming this barrier.

Post on or before Day 3:

  • (1)  an evaluation of the use, or lack thereof, of EBP in a recent clinical experience.
  • (2) Identify which aspects of the care delivered, if any, were based on evidence and provide your rationale.
  • (3) List your background questions and PICOT question about this nursing topic.
  • (4) Critique how the policies, procedures, and culture in your organization may hinder or support the adoption of evidence-based practices.
  • (5) Identify the barrier you selected from the article and explain how this barrier could be overcome within your organization

Identify the pathophysiology of the associated alterations, including the normal and altered cellular function. Consider both intra- and extra-cellular changes that occur.

Parkinson’s disease: describe the patients genetic, gender and age factors related to Parkinson’s disease. Reflect on how these factors might impact this disorder, as well as potential associated alterations and symptoms.

Identify the pathophysiology of the associated alterations, including the normal and altered cellular function. Consider both intra- and extra-cellular changes that occur.

Identify two (2) advantages and two (2) disadvantages of technology as they apply to the perinatal setting. Explain your answer.

The use of technology is increasing in all health care settings.

  • How does technology affect patient outcomes?
  • Identify two (2) advantages and two (2) disadvantages of technology as they apply to the perinatal setting. Explain your answer.
  • Discuss strategies the Registered Nurse can employ to reverse the two identified technology disadvantages. Provide a rationale for each strategy indicating how it results in the provision of safe, quality care in the perinatal setting. Include a reference from the literature to support the information provided.
