Describe the sources of data needed to conduct the two measures.

quality improvement initiatives including NCQA’s HEDIS® measures. Health plans and physicians must ensure they are meeting standards set by the accreditation agencies, such as NCQA. As a physician practice manager for Dr. Jones, you have just conducted a mock survey of the patient chart data. The data shows that your physician practice is not meeting standards for two HEDIS® measures.

  • Choose two HEDIS® measures (from the list below either a, b, c, d, e or f) that must be implemented in a physician practice to improve patient outcomes.
  • Describe the sources of data needed to conduct the two measures.
  • Using one of the quality improvement models (Lean, PDSA, or Six Sigma), explain how you would use the model to implement the two chosen HEDIS® Measures.

Your initial post should be 250-300 words and utilize at least one scholarly source library

discuss your opinion regarding why costs have continued to rise without improving quality. List two examples that illustrate your point.

The Department of Health and Human Services has oversight of several agencies (i.e., FDA, CDC, AHRQ, NIH, CMS) that regulate health care in the United States. Regulation encompasses insurance plans, cost, research, safety, all in the name of delivering quality care in a cost effective manner. These agencies are responsible for monitoring compliance and enforcing legislative mandates. However, the debate continues on government regulation and its effect on ensuring quality care. After completing this week’s reading, review the following articles listed below, which were published 11 years apart. Analyze the cost-quality paradigm noted in the articles. Considering the many governmental mandates and regulations to reduce costs and ensure the delivery of quality care that have been implemented over the years, discuss your opinion regarding why costs have continued to rise without improving quality. List two examples that illustrate your point.

Your initial post should be 250-300 words and utilize at least one scholarly source from the Library

Summarize the findings from the articles you selected for your literature review. Describe at least one nursing practice that is supported by the evidence in the articles. Justify your response with specific references to at least 2 of the articles. Please refer to the articles below:

Course Project: Part 3—Translating Evidence Into Practice

In Part 3 of the Course Project, you consider how the evidence you gathered during Part 2 can be translated into nursing practice.

Now that you have located available research on your PICOT question, you will examine what the research indicates about nursing practices. Connecting research evidence and findings to actual decisions and tasks that nurses complete in their daily practice is essentially what evidence-based practice is all about. This final component of the Course Project asks you to translate the evidence and data from your literature review into authentic practices that can be adopted to improve health care outcomes. In addition, you will also consider possible methods and strategies for disseminating evidence-based practices to your colleagues and to the broader health care field.

To prepare:

  • Consider Parts 1 and 2 of your Course Project. How does the research address your PICOT question? PLEASE REFER TO FILES ATTACHED BELOW
  • With your PICOT question in mind, identify at least one nursing practice that is supported by the evidence in two or more of the articles from your literature review. Consider what the evidence indicates about how this practice contributes to better outcomes.
  • Explore possible consequences of failing to adopt the evidence-based practice that you identified.
  • Consider how you would disseminate information about this evidence-based practice throughout your organization or practice setting. How would you communicate the importance of the practice?

    To complete:

    In a 3- to 4-page paper:

    1) Restate your PICOT question and its significance to nursing practice. My PICOT question is: does hand washing and appropriate staff dressing among the surgical ward nurses reduce cross infection during patient management?

    2) Summarize the findings from the articles you selected for your literature review. Describe at least one nursing practice that is supported by the evidence in the articles. Justify your response with specific references to at least 2 of the articles. Please refer to the articles below:

3) Explain how the evidence-based practice that you identified contributes to better outcomes. In addition, identify potential negative outcomes that could result from failing to use the evidence-based practice.

4) Outline the strategy for disseminating the evidence-based practice that you identified throughout your practice setting. Explain how you would communicate the importance of the practice to your colleagues. Describe how you would move from disseminating the information to implementing the evidence-based practice within your organization. How would you address concerns and opposition to the change in practice?

This part of the Course Project should be combined with the other two components of the Course Project and turned in as your Portfolio Assignment for this course.

Note:  In addition, include a 1-page summary of your project

Application
Community Assessment

In this week’s media presentations, Ms. Marren and Dr. Fernando Guerra discussed the importance of community assessment when determining the needs of the community and health promotion or intervention programs. In addition, Dr. Sattler examined the influence of the environment on the health of a community, along with occupational health concerns.

Community assessment is one of the three core functions of public health nursing and is the process of thinking critically about the community. It involves a logical, systematic approach to identify a community’s needs, problems (including environmental and occupational problems), and strengths and resources. Assessment of the health of a community can begin with a simple windshield assessment: a drive through the community noting strengths and problems. Community assessment also includes data gathering through examining vital statistics, and the demographics of the community.

To prepare for this Application, review the media presentations for this week and Chapter 18 in the textbook on community assessment. In addition, use Table 18–5 on page 410 in the course textbook to guide your windshield assessment. Incorporate your windshield assessment with your Application.

Conduct an assessment of the community in which you live, including a windshield assessment. Then, write a 3- to 4-page paper that includes the following:

  • Demographics
  • Vital statistics
  • Environmental issues
  • Occupational health hazards
  • Vulnerable populations
  • Resources available in the community
  • Other pertinent information about the community
(Incorporate information from the windshield assessment into the paper.)

Identify one evidence-based behavior change technique to promote health in this population.

Week 4:  Consider Levels of Nursing Intervention for key health concerns in your community.

Develop nursing interventions at the community and system level of care aimed at primary prevention for population of interest/at risk.
Draft interventions using culturally competent assessments and interventions to address primary prevention (see table below). Interviewing family or health care professional involved with the population at risk may help you create a successful plan. Support your ideas with the scholarly literature. (Do not include the individual level of nursing care.)

  Nursing Interventions/Services
Level of Prevention Individual / Family Community System
Primary This is an important  area to address, but do not focus your project on the individual Focus of Project Focus of Project
Secondary Do not focus on individuals  for this  project You may include secondary prevention in addition to primary prevention You may include a focus on secondary prevention in addition to primary prevention
Tertiary Up to now most of your practice has been focused at this level of care. (Acute Care) In this course you will focus on primary prevention.

Also, consider the following as they relate to your chosen population-based health problem.

  • Services provided by other health care providers
  • Lack of services
  • Cultural beliefs of community (group)
  • Health/illness beliefs
  • Values, attitudes, health behaviors and practices

Please address the following:

  • Provide a few examples of community resources that need to be in place to assist your population group’s problem.  What resources are missing?
  • Are there any cultural-based related issues regarding your group that should inform your approach to care?
  • Identify one evidence-based behavior change technique to promote health in this population.
  • Suggest one evidence-based intervention to address the health problem for your population of interest.

Describe the type of care this option provides. Consider the following: Who is the target client?

This week we discuss the levels of care in the U.S. healthcare system. There are three delivery mode options. The delivery mode you will be discussing depends on the first initial of your last name. See the table below.

Guided Responses: Those who are assigned to Delivery Mode 1 will respond to those who responded to Delivery Modes 2 and 3, Delivery Mode 2 will respond to those in Delivery Mode 1 and 3, and Delivery Mode 3 will respond to those in Delivery Modes 1 and 2. Review the six concerns listed in each of your classmates’ discussions. Select two concerns within each post and examine how stakeholders in their specific delivery mode might address two of them.

Students with last names starting with A–H are assigned to Delivery Mode 1—long-term care—throughout this discussion; submit rebuttals to each of the other modes.

Delivery Mode 1: Long-Term Care (last names A-H)
Examine this mode of delivery and address the following elements in your discussion:

  • Describe the type of care this option provides. Consider the following:
    • Who is the target client?
    • What professionals work within that mode?
    • What does this mode offer in terms of care?
    • What are the expected outcomes?
  • Discuss at least two specific issues or concerns that affect the delivery of care for each of the following categories listed below (a total of six concerns):
    • legal and regulatory
    • ethical
    • social

Guided Responses: Those who are assigned to Delivery Mode 1 will respond to those responding to Delivery Modes 2 and 3, Delivery Mode 2 will respond to those in Delivery Modes 1 and 3, and Delivery Mode 3 will respond to those in Delivery Modes 1 and 2. Review the six concerns listed in each of your classmates’ discussions. Select two concerns within each post and examine how stakeholders in their specific delivery mode might address two of them.

Final Presentation: Part I

Look ahead at the Final Presentation directions in Week Five. The assignment this week is Part I of that presentation. For this assignment, Part 1, you will provide an overview of the U.S. healthcare system. Follow the instructions below to complete this part of your Final Presentation:

Introduction: Include a title slide. Next, include an introductory slide where you will identify yourself and describe the nature of this presentation.
Part I (of the Final Presentation): The U.S. healthcare system. Define the U.S. healthcare system by addressing the following:

    • Describe the history of the U.S. health care system. Include at least three of the revolutionary factors (e.g., the teacher’s union in Dallas, Texas; The American Medical Association; Civil War, first Marine Hospital, etc.). Chapter two in our textbook discusses the evolution of our healthcare system and is a good resource for this part of the presentation.

    • Identify at least one major development from each of the following: financial, legal, ethical, regulatory, and social [e.g., consumer demand] that transformed the system into what it is today. You may want to revisit the healthcare timeline.

  • Differentiate the stakeholders and their roles (e.g., healthcare professionals, clients [patients], health insurance organizations, government, colleges, and health care agencies
    • Include the positive and negative contributions of how they affect the health care system).

Creating Part 1 of the Final Presentation for the Week 3 Assignment
Part 1 of the Final Presentation must:

    • Be 5 to 7 slides (excludes the title slide and reference slides) and be formatted according to APA style as outlined in the Ashford Writing Center.

  • Be presented using Microsoft PowerPoint slide presentation.  You have a choice of one of the following options for adding the narrative to your presentation. Option 1 is using voice or Option 2 is adding speaker notes to each slide.
      • Option 1: Recording your voice on each slide. You can view the tutorial using this link: adding and recording voice to each slide. You can also click on the following links to view the Accessibility Statement and Privacy Policy. If you choose this option, you must include your APA formatted citation in the speaker note section of the PPT as well.
        • Note: if you use the voice option, make sure your recordings are clear and concise. It is also important to know that adding voice narrations that are too lengthy and adding graphics may cause your PPT file to be very large (megabytes). You may have trouble loading it to Waypoint. Revisions may be necessary if this occurs.
        • Speaker notes are the typed notes that appear below the slide that complement the presentation slides. Whereas the slides will have short bulleted items, the speaker notes will be more detailed. They are essentially what the presenter would say during the presentation to explain each of the bulleted points on the slide. Therefore, it is important that the speaker notes are concise and detailed when explaining the bullet points.
            • It is recommended that PowerPoint Slides contain no more than five bullet points and should not contain more than 5 – 7 words each. Do not type paragraphs or long sentences on the slide. The information that explains each bullet point is conveyed via speaker notes or by recording your voice to each slide.
          • It is required that the Microsoft PowerPoint presentation include the use of a voice or speaker notes. If you choose to use the voice capabilities within PowerPoint, you must include your citations in the speaker notes for each slide.
        • Include a title slide with the following:
          • Title of the presentation
          • Student name
          • Course name and number
          • Instructor name
          • Date submitted
        • Address the sections in the order outlined above (Introduction, Part I, II, III, and IV).
        • Present the issues with critical thought.
        • Use at least two scholarly sources (none of which should be media commentary, at least one should be from the Ashford University Library).
        • Document all sources in APA format (including graphics, charts and pictures that may be used within the presentation). Wikimedia Commons is a recommended source for creative commons images (to view the privacy policy, click here).
        • Include a separate reference slide formatted according to APA style as outlined in the Ashford Writing Center.

If you are unable to access the technology for this assignment due to a documented accommodations on file with the Office of Access and Wellness, inform your instructor as soon as possible.

Carefully review the Grading Rubric for the criteria that will be used to evaluate your assignment.

60 What are social, behavioral, and cultural determinants? How do social, behavioral, and cultural determinants impact public health?

260 What are social, behavioral, and cultural determinants? How do social, behavioral, and cultural determinants impact public health? Present specific examples within your answer as well as justifying your rationale with evidence. Respond to three other class members’ posts specifically peer-reviewing their answers.

260 Very wealthy or influential public figures (such as Bill Clinton and Bill Gates) have become major funders in the global health arena, providing millions to developing countries and/or international organizations. How do you think this will influence the future of public health? What benefits and challenges may develop?
