Explain the significance of monitoring the prevalence of MDR-TB among Category II pulmonary tuberculosis patients.

Assignment 2—Effective Management of Tuberculosis

Multidrug-resistant tuberculosis (MDR-TB) has emerged as a global public health concern. Tuberculosis (TB) is contracted by breathing in air droplets from a sneeze or cough of an infected person. TB primarily attacks the lungs; however, it can spread to other parts of the body, especially if left untreated. The drug treatment regimen takes approximately 6 months or longer, so many patients become resistant to the medications after a certain period.

From the assigned readings this week, review the following article:

  • Sharma, S. K., Kumar, S., Saha, P. K., George, N., Arora, S. K., Gupta, D.,  . . . Vashisht, R. P. (2011). Prevalence of multidrug-resistant tuberculosis among Category II pulmonary tuberculosis patients. Indian Journal of Medical Research, 133(3), 312–315. (Web of Science Accession Number: WOS: 000289322400013)

Write a 3–5-page paper in Microsoft Word format that addresses the following:

  • Explain the significance of monitoring the prevalence of MDR-TB among Category II pulmonary tuberculosis patients.
  • Describe the methodology of the study in the above journal article and how the study sample was selected.
  • Analyze the study’s findings as they relate to the effective management of tuberculosis.
  • Summarize the results of the study.
  • Justify how the results of the study can improve rapid detection of MDR-TB.

Support your statements with appropriate examples and scholarly references.

3) Identify the change management strategies that you would use to facilitate a successful implementation. Identify key resources that you would need to promptly overcome potential barriers.

The Committee on the Robert Wood Johnson Foundation Initiative on the Future of Nursing, at the Institute of Medicine (2011) notes that “the nursing profession is the largest segment of the nation’s health care workforce.” Nursing’s three million–plus members are known for their ability to react quickly and efficiently on the front lines of patient care. This agility is lost, however, when the nursing profession fails to evolve with the field of information technology. Have you witnessed any unintentional barriers that have excluded nurses from quickly adopting and accepting informatics systems?
Those nurses who apply strategies developed by change management experts can help expedite the adoption process by positively addressing staff concerns and implementation challenges. In this Discussion, you explore change leadership strategies through a role-playing activity.

  • Review      the Learning Resources, focusing on the findings of the TIGER Leadership      Collaborative.
  • Consider      the TIGER Nursing Informatics Competencies model from a leadership      standpoint.
  • Engage      in the following role play: Your practice setting has appointed you to      lead the implementation of a new information technology initiative. As you      plan for this undertaking, you consider the various challenges that this      implementation may bring to your practice.

  • Select       an information technology initiative that your practice could benefit       from (i.e., a new information system, point-of-care service, robotics       machinery, mobile devices, glucometers, smart pumps etc.).
  • What       change management strategies might you utilize to facilitate a quick and       successful implementation?

1) Briefly identify your practice setting (hospital setting) and the information technology initiative you selected. Provide a rationale for your selection. (i.e., a new information system, point-of-care service, robotics machinery, mobile devices, glucometers, smart pumps etc.).

2) Evaluate the potential barriers to implementing your hypothetical initiative.

3) Identify the change management strategies that you would use to facilitate a successful implementation. Identify key resources that you would need to promptly overcome potential barriers.

4) Appraise the leadership strategies that you would employ to establish a culture that supports the new information technology initiative. Reference the TIGER Informatics Competencies where appropriate.

Outline how the study’s qualitative data collection and analysis did, or did not, promote rigor, provide scientific or systematic scaffolding, and/or generate a more thorough analysis of the research topic.

Which method of research provides the best evidence for nursing practice? Is there a place for both quantitative and qualitative research in evidence-based practice? How do these research methods improve patient outcomes?

Return to and reflect on your thoughts and postings from Week 1’s Discussion on the respective characteristics, strengths, and limitations of quantitative and qualitative research. Consider the uses of each type of research in the health care field. Also reflect on the amount of quantitative versus qualitative research that exists in the health care field in particular. There is an impression among some researchers that qualitative research is inferior to, easier than, or not as rigorous as quantitative research.

In this Discussion, you consider the idea of rigor, or thoroughness and scope of study, with regard to qualitative research. You examine the methods of qualitative research outlined and recommended in this week’s Learning Resources and how they aim to create standards of rigor by which to assess qualitative studies. You also have the opportunity to assess an article of your choice in terms of rigor and recommended methods of qualitative data collection.

To prepare:

  • Consider      your readings about and understanding of quantitative and qualitative      research. If you had to choose, which type of research (quantitative or      qualitative) do you think is more rigorous and why? Do you think it is      useful to make such generalizations and comparisons?
  • Locate      an article describing a qualitative research study related to a health      care topic (see attached PDF).
  • Formulate      a research question to address the problem and that would lead you to      employ correlational statistics.
  • With      information from the Learning Resources in mind, critically analyze your      selected study. Ask yourself: How rigorous was the study in terms of the      researchers’ efforts, the data collected, and the conclusions drawn? What      might the researchers have done to improve the rigor?

Post a cohesive response that addresses the following:

1) Do you think there is one type of research (quantitative or qualitative) that is inherently more rigorous than the other? If so, identify which one and why. If not, discuss your reasoning.

2) Post a brief summary of your research article analysis and the correct APA citation for the article (See attached PDF).

3) Outline how the study’s qualitative data collection and analysis did, or did not, promote rigor, provide scientific or systematic scaffolding, and/or generate a more thorough analysis of the research topic.

Analyze the challenges Medicare faces in terms of improving access to care, quality of care, and reducing the cost of care.

MMA was signed by President Bush in 2003. The MMA brought about sweeping changes that increased enrollment and improved access to Medicare Plans. Similarly, the PPACA was signed into law by President Obama in 2010. The law is expected to significantly increase access to Medicaid for the uninsured, low-income families, and individuals. Medicaid managed care has been implemented by most states, mainly because of rising costs, plan quality, access to providers, and the decrease in state revenues and federal matching funding. Medicare has explored managed care for the same reasons: to reduce costs, improve quality and improve access to providers. Using the South University Online Library or the Internet, research the following two topics and write a paper evaluating the impact of these policies on Medicare and Medicaid managed care.

You need to research on the following two topics:

  • Medicare and managed care
  • Medicaid and managed care

Based on your research, answer the following questions:

  • Write a summary for each topic tying together the information learned about that topic.
  • Analyze the challenges Medicaid faces in terms of improving access to care, quality of care, and reducing the cost of care.
  • Analyze the challenges Medicare faces in terms of improving access to care, quality of care, and reducing the cost of care.
  • Draw conclusions about the future of managed care in each program—will it grow or decline? What will be the challenges for success?
  • Select at least four reputable articles per topic that address the challenges facing Medicare and Medicaid managed care.

Based on your research, summarize your findings on the selected topics and compile your observations in a 5 page Microsoft Word document.

Support your responses with examples.
