A leadership strategy that the advanced practice nurse can incorporate to improve quality and cost effective care is to engage in shared decision-making, in which the patient and clinician collaborate on care plans together. A theoretical model on medical decision-making is categorized into four types according to the role of the medical service provider: paternalistic, informed, agent, and shared (Bae, 2017). The shared model is different from the other listed models in that two-way information communication occurs only in it (Bae, 2017). A provider that practices shared decision-making well is able to communicate effectively, preserve patient autonomy, encourage patient decision-making, and provide patient centered care.

With the incorporation of shared decision-making by the advanced practice nurse comes an expected increase in patient satisfaction with care, outcomes, and trust in the provider, along with a decrease in medical costs. Shared decision-making increases patient satisfaction by allowing a forum to voice their preferences and understand the risks and benefits associated with their decisions. Patients that participate in shared decision-making have been found to be more knowledgeable about their conditions compared to control groups and reported higher overall outcomes and satisfaction with treatment (Fisher et al., 2018). Employing this technique in practice can also help patients establish trust with their providers and helps the provider engage better in communication with their patients. This enhanced trust and respect between provider and patient may create a correlation with patients adhering to their plan of care. Shared decision-making can also decrease medical costs. Preference sensitive conditions, such as treatments for arthritis or back pain, allow the well informed shared decision-making patient to choose less invasive and less costly interventions if appropriate.

As future advanced practice nurses it is important that we begin to learn and practice leadership strategies to improve quality and cost effective care, such as shared decision-making.
