1C) What is the health-related behavior change GOAL you plan to implement in this project and how will
reaching this behavior change goal potentially affect the problem / condition that you identified and
affect your overall wellness.
Cite at least (2) references, from 2 different sources, in the text of your response to question #1 (author,
date). A full citation for these references should also be included in the final reference page of your
paper. One full page is an appropriate length for responding to question #1.

After the Question 1 paper, on the next page, insert your 1 wk. post-change bar graph. Below that,
insert your 9 week behavior change bar graph. Do not include the activity logs – just the bar graphs.
#2 – Question 2 Examine your pre-change, post-change, and total project behavior charts:
2A) Describe and explain the results of each chart.
2B) Discuss the differences and / or similarities you see in your behavior patterns among all three of the
charts.
About one half page is an appropriate length for responding to question #2.

#Question 3 Evaluate how successful you were in reaching your goal:

3A) Were you successful? Explain why you achieved or why you did not achieve success.
3B) Critique and identify at least one personal strength and one personal weakness that had an effect on
your success / lack of success in meeting your goal.
3C) Critique and identify at least one strength and one weakness in the way you designed your project
that contributed to your success / lack of success in meeting your goal.
One full page is an appropriate length for responding to question #3.

#Question 4 Choose a new behavior change project that you would like to implement in the future. This
behavior must be different from the one identified from the one identified in question #1.

4C) What is the health-related behavior change SMART GOAL you plan to implement in this new project
and how will this behavior change goal potentially affect the problem / condition that you identified and
improve your overall wellness. Provide a resource showing your new behavior change will help the next
problem you’ve identified.
