Develop a list of monitoring indicators you can use to track the progress of your project. These
indicators should be presented as a bulleted list that includes both the activity and the indicator. For
example:
 Activity: Distribute educational brochures about the importance of brushing and flossing to all
students in Fairfax County Public Schools.

You should have two activities based on what you identified in your program narrative. You should have
one monitoring indicator for each activity.
You must also develop two evaluation questions (based on the activity and related program objective)
Give a very brief description of how you would collect the data to answer these questions. For example:
 Evaluation question: Did students who participated in an in-class educational session report
that they were more likely to brush their teeth twice a day?
o Data Collection: The program team will distribute a survey that measures tooth
brushing intention before the session. One week after the session they will return and
distribute the same survey and measure the change in intention.

present them in paragraph form the assignment will be returned to you and counted as late until you fix
it.
This assignment should be 1 to 2 double spaced pages in length.
