Instructions: Apply this assignment based on your own experience as a patient using telehealth. You will be writing from the perspective of the patient. 

1.identify the hospital’s telehealth policy and create a teaching tool that is compliant to the policy and generic to the work setting and client/staff population, include policy in presentation (person & environment)

2.create an authentic educational tool such as a care plan, triage algorithm, discharge plan, etc. that complies with the nursing process (ADPIE). That is engaging or interactive for the target population and is measurable 

3.this plan will be a visual presentation that can be translated to a telehealth format (nursing)

4.include goal and objectives that are SMART
