Rodney is a 62 y/o male recently discharged after a stroke resulting in right sided weakness to the leg and arm. Rodney lives alone but has family members that come by and check on him throughout the day, however, he is having trouble performing daily tasks such as cooking meals and getting dressed. The patient’s family now has concerns that he is not able to live alone safely so at his PCP follow up they discuss their concerns with the doctor. The doctor strongly recommends that he see physical therapy as discussed at his hospital discharge as well as occupational therapy. The doctor also recommends the patient be assigned to a case manager to help to coordinate home health services or transfer to a nursing facility.
