A tool that my practicum site utilizes for gathering evidence supporting the effectiveness of programs or initiatives is the American Diabetes Association websites for guidelines and resources. This website provides detail and resources on diabetes and the comorbidities that may be associated with diabetes and how to control and manage the illness, such as kidney disease and eye health. The website also provides guidelines on weight loss, mental health, medication & treatment information, devices & technology, recipes & nutrition for a healthier lifestyle, and more (American Diabetes Association Organization, 2021).

An explanation of the methodology of how the tool was used to evaluate a program

The Nurse Practitioner would see a patient and diagnose them with diabetes based on the lab test results. The nurse practitioner will then evaluate the individual’s lifestyle and create a diabetic plan. This plan will be created based on the resources and guidelines that are on the American Diabetes Association website. Afterward, a consultation with the patient and the Nurse Practitioner will occur to go over the plan, goals, and expectations and the organization would know if the plan were effective when the patient achieves and maintain a normal blood glucose level and BMI.

One barrier in applying the data to improve practice

A barrier in applying the data to improve practice is the data is that this information is a general standard guideline and might not be effective for everyone because each person’s body and lifestyle is different. Therefore, the physicians need to evaluate the individual’s health and lifestyle and use the tools or data as a guide to creating a specialized diabetic plan specific to the patient.
