2. Give three (3) examples of both subjective and objective data. 

Include pertinent positive and pertinent negative data associated with each. Provide rationales for your examples.

 

Subjective data is what the patient says about himself or herself during history taking.

Objective data is what you as the health professional observe during the physical examination.

 

A pertinent positive is an element of a patient’s history which aids in diagnosis because the patient affirms it is present, or it is present upon physical exam. For example, the patient complains of shortness of breath and cough.

A pertinent negative is an element of a patient’s history which aids in diagnosis because the patient denies it is present or it is not present upon physical exam or in lab work up. For example the patient complains of SOB and cough, but there is no fever or sputum, (rule out pneumonia).

3. Discuss how the Foundation of Knowledge Model and the science that supports nursing informatics can guide nursing practice, education, leadership, and research.
