Working in healthcare automatically exposes you to and demands interactions with multiple healthcare disciplines. Each professional discipline you encounter in healthcare has its own code of ethics, whether social workers, physical therapists, physicians, etc. Of course, you know that nursing has its own Code of Ethics written by the American Nurses Association. It should come as no surprise, then, when you read public health also has its own code of ethics. However, it is not the writing of the ethical codes that is difficult. What is difficult about ethical codes is application. Whether discussing ethics from a personal dilemma perspective or a broad social context, it is doing what the ethical code says you are to do that is difficult. Therefore, in this discussion, you will become more familiar with the Public Health Code of Ethics and then tackle the task of developing your own personal position on the topics of social justice and ethics.

Upon successful completion of the course material, you will be able to:

Background Information

Ethical Case Study

You are a public health nurse in a small, rural town. A number of migrant farm workers and their immigrant families reside in the community, and many of these families live in the Summerton District public housing development.

Because of a recent increase in children with elevated blood lead levels in the Summerton District and surrounding census track, the public health department initiates a thorough inspection of this region for lead-based paint. In this process it is discovered, illegal immigrants have been living in abandoned homes. The public health department was aware these homes were contaminated with lead paint and were boarded up until the rental company decided what to do, based upon their limited options. The homes are owned by a low budget rental company which did not want to expend the revenue to address lead abatement. The public health department was not aware of the illegal immigrants having moved in. The children with elevated lead levels have been playing with the illegal immigrants. During the process of lead abatement, the immigrant families, who have lived there for decades, are displaced to other areas of the small community.

An additional item of concern is an increased incidence of cystic fibrosis among children in this census track over the past two years. As knowledge about the impact of genomics expands, ethical implications regarding population-focused health practices at the primary, secondary, and tertiary levels of prevention will increase. The public health department has developed an educational support group for families and is collaborating with a regional children’s hospital to raise awareness and establish a screening initiative for cystic fibrosis.

Prepare to discuss the following prompts:

Within the context of the Public Health Code of Ethics (page 132 in the Public Health Nursing: Population-centered Health Care in the Community textbook), consider the two main events in the above scenario (displacement of families due to lead abatement and presence of illegal immigrants.) Select two of the principles from the Public Health Code of Ethics and discuss their application to this situation. Utilize a minimum of two current scholarly sources, correctly cited and referenced, to support your information.

When the renters have to leave, it is based on the principle of beneficence: to do good. How do you feel this initiative could be implemented so that the renter’s autonomy, which allows them to choose where to live, can be supported? When considering the application of ethical principles in public health and/or population-focused practice, describe how the principles of beneficence and autonomy are at odds with one another.

Identify and discuss one ethical implication of the prevention strategies that may impact the public health initiatives regarding cystic fibrosis in this community. Relate your discussion to the role of genetics in cystic fibrosis.

Based upon the public health tenets regarding social justice, ethical principles, and Christian values, provide a succinct personal position statement regarding the ethical care of at-risk populations (two to three sentences). Be sure to address each of the three foundational components of social justice, ethical principles, and Christian values.
