Kindly can u help me with my project (allergic alert) , the project should be in this format [Description , Rational/ why this projects, proposed measure , people to be involved ]

the project will help to improve the medication safety to the patients.

I choose this topic cause , when I’m reviewing patient file I noticed that the patient is allergic to X medication, and NO one [Physician , Pharmacist ] notice that the patient allergic. however, I counsel the patient to make sure if he allergic then he told me that he is allergic but take the X medication and give it to his family to use it ! 

so the point of this project to improve the safety at hospital system by redesign the allergy alert also cost saving [especially in this situation  ]
