Write a letter for exemptions for taking Covid shot at work? Like opting out of Covid-19 shot for spiritual and religious reasonings 

I want mine geared towards vaccination. I am ok with wearing the mask to protect myself. I reside in Michigan and we have both religious and philological exemptions. My religion is spiritualist.
