Alcorn State University Analysis and Prioritization Community Health Assessment

 

PURPOSE

The purpose of this assignment is to utilize the health promotion process steps 1-2 by:

  • Conducting a community health needs assessment for a specific population within the county where you live or practice nursing.  
  • Collecting health behavior and health outcome data for the community.
  • Identifying a priority health need for the population based on your analysis of the data.
PREPARE
  • Background primer: Assume you are a community health nurse employed by the county where you live or work as a nurse. You are tasked with developing an annual health promotion initiative that aligns with one of the Healthy People 2030 objectives for your state/county.
  • Each part builds on the next assignment. You will use the assessment data collected for the selected location and population to inform the development of your plan, including measures for evaluation.
  • You will be required to submit previously completed assignment parts with each new part. 
    • For example: 
      • For Part 1, submit Part 1
      • For Part 2, submit Part 1 and Part 2
      • For Part 3, submit Part 1, Part 2, and Part 3
  • You may prepare your assignment in bullet summaries, a table, or narrative format. Please include descriptive headings to facilitate evaluation by your instructor.
DEVELOP
  1. Assessment
    1. Define the Population
      1. Geographic location: This is the city or city or county AND state where you live or practice nursing
      2. Identify population characteristics: These are the key characteristics that describe the population intended to benefit from your health promotion plan. We will use the three age groups outlined by the Leading Health Indicators (LHIs), from the Healthy People 2030 (HP 2030) national health objectives. LHIs are a small subset of high-priority HP 2030 objectives. The categories include: 
        1. All ages
        2. Children and adolescents
        3. Adults and older adults
      3. You may include additional population characteristics as you refine the direction of your plan. Considerations for further refinement include characteristics that increase the population’s risk for disease or injury, such as gender, social, cultural, or economic factors. 
    2. Collect Population Health Data
      1. Research health behavior data for your county and state using the County Health Rankings & Roadmaps database.
      2. Begin by accessing the County Health Rankings & Road Maps database at:  https://www.countyhealthrankings.org/app/

        2. Select your county
        3. Review the Health Outcomes AND the Health Factors Rankings
      3. The Social Determinants of Health (SDoH) framework is used widely across health care disciplines including public health, private and nonprofit health care institutions, epidemiology, and health policy researchers and advocates. Use the six framework categories to organize assessment data relevant to your population and topic. Not all characteristic examples will apply to your population/topic, but your assessment should include data from each of the six categories.

  2. Analyze & Prioritize
    1. Analyze the Data
      1. In two paragraphs or less, prepare a brief report summarizing the most significant health outcome and health behavior concerns for your county. 
      2. Address the following questions/criteria within your summary:  
        1. What is the overall health ranking of your county compared to the rest of the state or nation?
        2. Discuss possible relationships between the health behavior, social & economic factors, and clinical care measures for your county and state.
        3. How might these relationships between health factors be influencing the health outcomes of your target population?
        4. How do the state health outcomes and health behavior rankings compare with other counties and the nation as a whole? 
    2. Prioritize the Community Health Need
      1. Identify one health outcome for improvement based on your assessment and analysis.
      2. Identify at least one Healthy People 2030 objective aligned with your priority.  (Interactive infographic for locating national health objectives)
      3. Summarize the significance of the priority selected in three to four sentences. Address the following criteria/questions in your summary.
        1. Describe the importance of the priority health condition for improvement. This may include the impact the health risk poses for the population’s overall health, quality of life, mortality, morbidity, and/or disability, health care costs, or other outcomes.
        2. Include at least one additional source from the literature to support your selected priority.
