Frameworks and models used to guide the implementation of evidence-based practices. There are many models and frameworks available to the advanced practiced registered nurse. However, as APRNs, we must choose the correct framework that will guide the implementation of the purposed EBP successfully. To perform this correctly, Dang, et al., (2019), purpose a vital set of factors that must be present in a framework, regardless of which model may be used. The authors explain these factors as:

·      Research Evidence- There must be adequate research evidence. 

·      Availability of Healthcare resources- Although there are great ideas does the organization can provide the resources for a successful implementation. 

·      Patient Preferences and Actions- Will the intervention be accepted by the patients? Will they be compliant? 

·      Clinical State, Setting, & Circumstances- In reference to patients, low income vs. high income, rural vs urban, etc. Will the patients be compliant with the new implementation?

·      Clinical Expertise- Does the new EBP require additional education or resources? Or specific personnel be hired? Is this something that the organization can do? I am sure that budgeting and finances play a major role in any new implementation (pg. 380-382).

These are important factors to consider when choosing a framework for implementation of any new EBPs. Choosing the correct model for your specific department or organization will depend on many factors, therefore it is important to choose the right model so that implementation of EBPs are successful. 

Biases can be formed when choosing frameworks or models for implementation of EBP, as it can essentially manipulate research. O’Mathuna & Finehout-Overholt (2019) explain that sometimes biases are created during the follow up regarding the implementation of new EBPs. “… if people stop the intervention because of an adverse event and this is not followed up on or reported, the reported outcomes may mask important reasons for observed differences between experimental intervention and control groups” (pg. 135). This is important to note, as in my specific intervention, it is important to follow up with the patients/participants regarding having fallen. For example, would it be biased to not count an emergency department visit with a chief compliant of a syncopal episode and subsequent fall, when collecting my evidence for reporting to the organization?  It is still technically a fall.

Evidence-based practice (EBP) is the result of research and the implementation of that research on current practices. The purpose of using EBP is to improve the outcomes of care and improve the quality of care. As stated by Speroni et. al (2020, p. 98) “EBP models were used most frequently for education and training, nurse residency programs, and EBP and research fellowships.” So, it is important that we select the correct model so that we can apply it to the different avenues like education and training. Not all a EBP models are appropriate for the different research or development of EBP projects. Selecting the wrong EBP model will have an effect the outcomes of the research or EBP project thus potentially creating bias. It is important that research that is to be utilized as EBP is free of bias. If EBP is biased it will not be free of prejudice or there is a error that is introduced that encouraging an outcome over another. So, selecting the correct EBP model has an impact on the outcome of EBP research.

Evidence-based practice utilizes research and clinical findings to guide the nursing process (Wilson, et. al, 2017). Although there is plenty of scientific data stating why the implementation, or change in practice, is important there is still a barrier in implementation within the nursing practice (Wilson, et. al, 2017). Wilson (2017) states nurses may have difficulty implementing EBP because the research was conducted on a population that is not indicative of the population [they] serve. In order to obtain validity and promote implementation it is important to avoid bias when conducting research. Selecting the appropriate framework or model with help reduce bias in research. In order mitigate bias within research, one must have understanding of ones own feelings, emotion and motivation for conducting the research. Although frameworks and models guide research and implementation, if the wrong model is selected, a poor outcome may follow. A plethora of models are available to choose from. One may select a model that focuses on team decision-making while another may select that focuses on evaluating evidence. If a nurse leader selects “evaluating evidence” and the evidence is not applicable to the community, nurses may have a hard time respecting the research or find the desire to change their practice (Wilson, et. al, 2017). 

Understanding the desired outcome, demographics and one’s own thoughts/feelings are imperative when selecting the appropriate model or framework in research and implementation.
