Case Conceptualization

Case of Ka-Sean (Kress & Paylo, 2019, p. 141-142)

Ka-Sean is a 25-year-old African American lesbian. Currently, she resides with her partner, and she is enrolled in a counseling graduate program. Ka-Sean has an estranged relationship with her brother due to his non-support of her partner selection (Kress & Paylo, 2019, p. 141). The client’s various psychological impairments are “analysis paralysis,” incapacity to sustain concentration, constant thinking causing anxiety, allowing her thoughts to run away from her constantly creating a lot of doubt and questioning all things (Kress & Paylo, 2019, p. 141). The client is dealing with several emotional impairments from consumption of excessive anxiety; worries uncontrollably about things beyond her control, she fears the worst of everything, triggering concerns about her relationship as well that causes emotional and physical fatigue; distress and torment from her “flooding” thoughts of infidelity, unrealistic love, and judgment (Kress & Paylo, 2019, p. 141). Ka-Sean’s behavioral impairments consist of needing constant severe procrastination, she does not her own judgment, so she is always looking to others for decision making, inability to distribute tasks (Kress & Paylo, 2019).

The client has a long history of anxiety and had obtained treatment during the loss of her mother who passed away three years ago. Her given treatment for self-injury, severe anxiety, and sorrow (Kress & Paylo, 2019). Ka-Sean stated that she has since ceased her behaviors of cutting, pinching, and punching herself (Kress & Paylo, 2019, p. 141). The extensive anxiety that the client experiences have caused her to have multiple medical as well as physical problems consisting of “ “stomach issues, muscle tension, chronic restlessness, fatigue, “battles” of “tossing and turning,” difficulties sleeping, digestive issues, stomach ulcer, upset stomach, severe heartburn, bowel changes, heart palpitations, hands shaking, anxiety that stems chest tightness, and shortness of breath this happened about once or twice a month over a two-year span (Kress & Paylo, p. 141). Ka-Sean is currently on a prescription of alprazolam (Xanax) as needed to treat her high anxiety.

Ka-Sean’s support system consists of her sister, father, and her partner. Her brother is not supportive of her current decision to cohabitate with her partner, it caused a strain on the relationship, and they have not been in contact for some time now. The workforce has its onset of challenges for Ka-Sean because she cannot perform her duties without becoming overwhelmed and depleted by the demands of her making choices at work (Kress & Paylo, 2019). Overall, Ka-Sean is very intellectual, amusing, and empathetic to others, which causes her to pour more of herself into others than replenishing herself. Henceforth, she decided to pursue counseling and she is a natural writer but allows the possibility of rejection to keep her from seeking to have any of her novels published (Kress & Paylo, 2019, p. 142).

Case Conceptualization

Ka-Sean is a 25-year-old African American female. She identifies as a lesbian; and lives with her partner. Ka-Sean is the youngest sibling of three children. She hasn’t spoken to her older brother in years because he didn’t agree with her moving in with her partner. Ka-Sean’s primary support system is her partner, sister, and father. Ka-Sean struggles with anxiety in her daily; which contributes to many factors in her life (e.g. her relationships, health, the future, and school). Ka-Sean has a history of chronic restlessness, stomach issues, muscle tension, fatigue, and difficulty with sleep.

Ka-Sean is currently dealing with a medical condition associated with her daily struggle. Ka-Sean is a current graduate student and struggles with the course due to her anxiety. Ka-Sean lost her mother a few years ago during this time Ka-Sean went through a dark period in her life where she engaged in risky behaviors including self-harm. She no longer engages in those behaviors; however, in stressful situations, she feels less in control of herself.  Ka-Sean is currently taking alprazolam (Xanax) as needed.

Diagnostic Impressions

F41.1 Generalized Anxiety

The rationale for Diagnostic Impressions

Based on the information given from the case about Ka-Sean she appears to be experiencing symptoms consistent with F41.1 General Anxiety. Ka-Sean meets the following Criterions Criterion A, B & C. She meets Criterion A as evident by Ka-Sean displays excessive anxiety and worry (apprehensive expectation), occurring more days than not for at least 6 months, about a number of events or activities (such as work or school performance).  

She meets Criterion B as evident by Ka-Sean having difficulties controlling her worry. She meets Criterion C as evident by Ka-Sean meeting 3 or more symptoms associated with the Diagnosis. She has experiences with the following; Restlessness or feeling keyed up or on edge, difficulty concentrating or mind going bank, and sleep disturbance. Functional Consequences of General Anxiety Disorder that is associated with Ka-Sean diagnosis are excessive impairs the individual’s capacity to do things quickly and efficiently as evident by Ka-Sean having a difficult time focusing and completing her school work in a timely manner.
