Getting Started 

Part Two of the Community Assessment involves the examination of the last five subsystems within the community. You will assess these elements independently. For the first time, you will introduce your at-risk population. This will involve researching information about the health issues of this at-risk population on a state, national, and global view. You will then assess the at-risk population in your community and compare the two. A new key informant should be interviewed and reported. Your field experience with the chosen at-risk population should also be reported.  

Upon successful completion of the course material, you will be able to: 

  • Conduct part two of a local community assessment, including the health and social services, education, politics and government, communications, and recreation subsystems. 
  • Interpret your findings from part two of a local community assessment involving the appraisal of the politics and government, recreation, communication, health and social service, education subsystems, and selected at-risk population.

Background Information 

As a ‘head to toe’ approach is used to assess the health of an individual, a structured and systematic approach is employed to assess the health of a community. As shared in Workshop One, for the purpose of this course, you will continue to use the Community-as-Partner Model to guide your community assessment. 

In this assignment, you will post your findings in Dropbox. This will include the systematic assessment process: the assessment of the health and social systems, education system, politics, and government systems, communication system, recreation system, at-risk population health status, and a new Key Informant interview. Review Key Informant: The Three W’s. 

In Workshop Five you will analyze your total findings to determine the health needs and strengths which are present in your community. At that time, you will apply the nursing process to the community. 

Instructions 

  1. Review the appropriate rubric to make sure you understand the criteria for earning your grade. 
  2. Download and review the Community Assessment Part Two Data Collection Tool.docx. Note the information that is mandatory for your assessment in each subsystem. 
  3. Based on previous reading assignments, conduct an assessment of your selected community (Chapter 11 E-Book, Community as Partner: Theory and Practice in Nursing). 
  4. Read pages 409 and 416 in Public Health Nursing: Population-Centered Healthcare in the Community. 
  5. View the optional video Unnatural Causes#3: Becoming American. 
  6. Begin the assessment of these five community subsystems using the Community Assessment Part Two Data Collection Tool: 
    1. health and social services, 
    2. education, 
    3. politics and government, 
    4. communication, and 
    5. recreation. 
  7. Utilize a minimum of five current scholarly sources, plus one new key informant interview to support your information and information you have gleaned from the meetings with your at-risk population. Review NUR-465 Resources. 
  8. Be sure to properly cite and reference your sources according to APA format (a key informant is cited as personal communication and not referenced). 
  9. Submit your Community Assessment Part Two by the last day of Workshop Four. 

Here’s part of the interview I was able to do before being dismissed due to an emergency situation. 

My informant for this interview is: 

Marcie Balford, Educator with Dekalb County School System. She has been an educator for the past 22 years. She informed me during our interview that there were a total of five deaths involving teens from the community, one of which resulted in death by suicide, two were results of motor vehicle accidents, one ruled as a gang related homicide, and the last death resulted from an accidental drug overdose.

She answered increased to both questions as a result of drug and alcohol paraphernalia being found in lockers and backpacks. 

What programs are in place to prevent alcohol, tobacco, and other drug use use among youth and young 

adults?  Alcohol & Substance Abuse Prevention Project (ASAPP). The ASAPP grew out of the Alcohol Prevention Project (APP) which began in 2011. The following are the 2017 OBHP State Substance Abuse Prevention Primary goals: 

1) Reduce the early onset of alcohol use among 9-20 year olds 

2) Reduce access to alcohol and binge drinking among 9-20 year olds 

3) Reduce binge drinking and heavy drinking among 18-25 year olds
