You are to select a specific disability as it relates to either chapter 10-14 (select one chapter) to complete the Assistive Technology/Consumer Service Plan. 

Items that will need to be included: 

I.  Develop a “Case Study” 

Include- 

 Name, age, race, gender, disability, psychosocial, education, vocational histories, etc. 

 The disability must be described in detail such as the onset of the condition (i.e., How, When, and Where did the disability occur?) 

 Must include detailed description of the condition/disability (i.e., functional limitation(s), and functional capacities) 

 You will be allowed to utilize the Cook & Hussey textbook, or any other resources that you feel will assist you in completing the activity. 

•  Indicate funding source(s) for the AT devices (See chapter 5) 
• II.  Identify the consumer’s needs using the HAAT model 

III.  Identify who (i.e., professionals) will be involved in the consumer’s AT service, and indicate each person’s role in the AT delivery process. 

• IV.  You have the choice of assessing the consumer’s need for AT in the context of a vocation (work), residential (home), or leisure/recreation (play). Select only one area!!! 
•  All vocation/job, residential, and/or leisure/recreational tasks must have a detail task analysis (i.e., steps involved in the activity and how the AT device will allow the person to perform the task). 
• At least three assistive technology devices must be recommended and 

the sources of the AT devices must be indicated (i.e., internet site, catalog, book, etc.)
