Student #1: Communication is a two-way process used to send a message between two or more people. It is the key to a successful group and ultimately to improving patient outcomes. One way to overcome communication barriers is to become effective at giving and receiving feedback (Huber, 2018).

A situation in which there was less than optimal communication between staff, includes a primary RN, respiratory therapist and OPO RN. The patient was an organ procurement patient, in this type of situation the OPO RN takes over complete patient care, including ordering medications and ventilation. The order administered to the primary RN was to place the patient on 100% Fi02 for 30minutes; every four hours, draw an arterial blood gas to measure lung capacity for organ placement. The primary RN relayed the information to respiratory, RT was a new traveler on the unit and their first time caring for an OPO patient. The RT left the patient ventilated on 100% for 8hours. The oncoming RN caught the mistake by assessing the ventilator settings during patient handoff. Although the results of the arterial blood gas were inaccurate, no detrimental damaged occurred to the patients’ lungs.

The lapse in communication could have been prevented by adhering to the orders and making sure delegation to RT does not get misconstrued by clarifying the orders and asking them to repeat the order back. There was no initial punishment involved, considering we all learn from the mistakes we make. Ways to combat this barrier include employing active listening and repeating what you thought you heard. This can be done by monitoring nonverbal cues or asking follow-up or clarification questions. It is also a good idea to avoid making assumptions (Huber, 2018).

Resolving communication problems quickly prevents the situation from becoming worse and disrupting work flow or spreading to include other employees. Poor communication occurs at every level. A recent communication order that I recently witnessed was when one of the nurses that I supervise was told that a medication order had changed. The original order was for twice a day, once in the morning and once in the evening. The evening dose was keeping the patient awake because of the type of medication that it was so it was changed to be given in the morning and in the afternoon, instead of the morning and bedtime. The nurse was told that the order had changed and she was aware of the change. The change was told to the nurse but not showed to her. The lapse could have been prevented by better explaining that the medication change had been made and what times it needed given. When the patient came in for his medication in the evening, he told her that he was still supposed to get it in the evening. Instead of going by the Emar for medication administration, she took the patient’s word and was administering it to him also in the evening. It turned out that the patient was lying to her and instead of receiving the medication two times daily, he was receiving it three times daily.

The nurse stated that she had been giving him the medication because of what the patient was telling her instead of going by the Emar. I ended up having to write the nurse up and give her a verbal warning for a medication error that could have been prevented if she would have paid attention to the Emar. The steps to improve communication is to make sure that the nurses are all going by the Emar instead of what the patients tell them. Most of the patients will be honest with you and not take anything extra, but there are always those few that like to play the system and see what they can get away with.

She was advised that maybe it was because of too much work without rest, and the solution was to take a break. She followed the internet medical advice and only realized that she had diabetes after her condition worsened and went to the hospital. The internet medical advice, in this case, misled the patient and caused her condition to worsen.

He took it upon himself to apply baking soda directly on his skin, in return caused a rash and allergic reaction. Luckily, I keep saline and Bactroban ointment in our medical supplies.
