Read the Norcross & Cannon (2008) article (It will be clear by what you turn whether you have read the article or not).  There is sage advice in this article that may prompt you to keep it for future reference – it is that good. 

)  create your own “Sample Request for a Letter of Recommendation” template.   You can leave the name of the professor blank, or include someone from  the CSUB psychology department.  Alternatively, you could write in an  employer’s name.  Make sure to format your the address just as shown in  the example (you could use CSUB’s address, formatted appropriately, if  you wish).  Then go on from there.   

You can input real information (if you wish) regarding a  program to which you plan to apply for graduate school, or a position to  which you would like to apply – or make one up.  This is your template.

Fill in courses if appropriate (but leave out grades to  maintain your privacy – you don’t need to share grades you’ve earned in  other courses with me)

List other activities that may be relevant

If you have taken the GRE and wish to list that, feel free (but, keep your scores private)

Finish off your template letter (leaving your personal details private from me)
